This survey is being conducted by UFCW Canada and has been developed by UFCW Canada disability justice advocates, both persons with disabilities and their allies. All survey responses are anonymous.

The purpose of this survey is to gain insight into the experiences of workers with disabilities in relation to accessibility and accommodation. This information will allow UFCW Canada to identify the main concerns of its members and staff. In doing so, it will inform the work of the UFCW Canada Human Rights, Equity, and Diversity department in establishing workplace rights and disability justice priorities. For this reason, you will be asked questions on demographics, workplace climate and employment equity priorities.

• you are a UFCW Canada union member who identifies as a person with a disability; or

• you are a UFCW Canada union member and an injured worker with a long-term or permanent disability, a caregiver of someone with a disability, or a disability justice ally.
